Authorization: I hereby grant Jose Luis Salon INC. permission to use my name and photographic likeness in all forms and media for advertising, trade, and any other lawful purposes, including reproductions for portfolio samples, self-promotion, professional competition, and stock/publication use. I understand that by submitting this form, I am not guaranteed a position/booking for this or any assignment. If selected by Jose Luis Salon INC., I will be notified by a representative to make arrangements to enter into a formal agreement for any shoot/runway show. I understand that this is a service trade shoot/runway show and no monetary compensation may be provided. Applicants under the age of 18 may submit their application online, but will be required to complete a paper application which will need to be signed by a legal parent or guardian. This application must be filled out prior to participation of any kind.
